Anyone who needs to have a USCIS form completed is welcome to book an appointment anytime. Patients are required to bring along their proper documentation, current immunization record, and a valid photo ID or passport on the day of the appointment. We do conduct additional testing and procedures to fulfill the requirement for the completion of the forms.
